Life Care Plan Review

Many referral sources are interested in requesting a file review for second opinion purposes; Discovery Life Care Planning & Vocational Research, LLC. is comfortable in providing such reviews and appreciates the need to maintain the confidentiality in such instances. An overview of the medical records is undertaken along with a review of the life care plan and any associated documentation. An oral conference is usually held in follow-up unless a written report is specifically requested. A life care plan review typically includes:

• Review and feedback regarding the disability involved
• Review and feedback regarding the life care plan submitted for review
• Feedback on the clinical practice guidelines and research literature used to support recommendations
• Recommendations regarding documentation that should have been included in the life care plan
• Feedback about whether the plan represents a well-coordinated, well-integrated, and well-supported plan of care
• Suggestions regarding alternative recommendations, as appropriate
• Identification of strengths and weaknesses within the life care plan
• Analysis of the proposed costs within the plan for consistency and substantiation
• Suggested approaches to questioning from a defense perspective

Research Literature Review

• Summary and analysis of research literature relative to the medical and rehabilitation issues involved in a case
• Identification and summary of relevant clinical practice guidelines, standards of care, and regulations impacting a case

Consultation with Members of the Treatment Team

• Development of questions and responses from members of the treatment team involved in a specific case
• Summary of recommendations of members of the treatment team 
• Development of deposition questions for treatment and consulting physicians as well as health-related professionals based on the clinical practice guidelines and research literature related to the specific disability and client involved

Rehabilitation Evaluation in Non-Catastrophic Cases

• Evaluation of the injured individual and provision of a written report detailing current limitations and rehabilitation needs
• Development of a narrative report and comprehensive rehabilitation plan for mediation purposes

Rehabilitation Evaluation in Non-Catastrophic Cases

• Small group instruction regarding life care planning can be provided as educational workshops for your group or organization
• Training in the sequelae of specific disabilities such as spinal cord injury, traumatic brain injury, amputation, and chronic pain. (This can include the application to life care planning, or can be limited to an understanding of the disability)
• Workshops developed for your specific needs in cases involving catastrophic and non-catastrophic injuries
